CPEx Applicant Information for Award, Review, and Feedback Program

Award Program Overview.
Colorado Performance Excellence offers awards to organizations in any of the
following segments: Manufacturing, Service, Small Business (less than 250 employees),
Health Care, Education, Non-profit, and Government.
The Colorado Performance Excellence Award criteria covers
seven areas: Leadership; Strategic Planning; Customer & Market
Focus; Measurement, Analysis, and Knowledge Management; Workforce
Focus; Process Management; and Organization Results.
There are eight steps to pursuing the CPEx award:
Decide to Apply: This is a serious commitment of time and resources
and should have support at the senior level of your organization. Some of

Attend an Introductory Awareness Workshop: CPEx has developed a
workshop designed to introduce you to the Colorado Performance Excellence
Criteria and Awards process.
Determine Award Level: Following the Baldrige model, CPEx has created
a four-level progression of award criteria.
- High Plains Recognition This level is for organizations
who are interested in self-assessment and the initiation of a journey
for performance excellence by completing an organizational profile. This
includes filling out a document which is a snapshot of the organization,
the key influences on operations, and the key challenges faced. The organizational
profile also describes the competitive environment, key strategic challenges,
and a system for performance improvement.
- Foothills Performance Award This level is for organizations
in the early stages of the journey to performance excellence. The application
includes questions on the organizational approach or how the organization
does things (processes or systems). Organizatons should apply at this
level if they have not previously done Baldrige assessments or do not
have expertise in understanding the complex linkages and alignments of
the Baldrige criteria.
- Timberline Performance Award This level is for organizations
that gained expertise in the Baldrige criteria and feel they are ready
for detailed discussion of their process. The discussion is expected to
include information on Approach, Deployment, Learning, and Integration.
- Peak Performance Award This level is for organizations
that have the expertise to apply for the National Baldrige Award. The
Peak Criteria include questions on Processes and Results (quantitative
results of key organizational metrics including trends and comparisons
to best in class and leading competitor organizations). The Peak Performance
Award Criteria is identical to the Baldrige Criteria. Organizations that
have had strong internal performance excellence programs and feel that
they are near to being a Malcolm Baldrige applicant may select the Peak
Performance level.
Complete the "Intent to Apply" form: Each applicant for the Colorado
Performance Excellence Awards must complete the Intent
to Apply form and return it along with a cover letter on company letterhead
from the most senior official from the organization. Also include check,
money order, or Visa, MC, or American Express card information for $100
to Colorado Performance Excellence, Inc. for Foothills, Timberline and Peak
applications. High Plains applicants should submit $25 with their Intent
to Apply form.
Complete Application and Organizational Profile: Preparing
your Application is a very significant undertaking requiring planning,
writing, and reviewing the application. The completed application, an application
cover page indicating organization, date, and application level, the organizational
profile discussed under each application level, and necessary application
fees should then be mailed to Colorado Performance Excellence. Twelve
copies are required for Foothills, Timberline and Peak applications.
Six copies are required for High Plains applications.
Site Visit: A site visit
is conducted as the final step in the evaluation process. Site visits are
required of all applicants at the Foothills, Timberline,
and Peak level. The applicant must agree before submitting an application
to pay reasonable travel, lodging and meal expenses for examiners on the site
visit.
High Plains applicants may receive a consultation instead of a site visit
and
must agree to pay expense of examiners/staff to attend a live consultation.
Receive and Review Confidential Feedback Report: When the feedback
report is received, your team should review the report for strengths and
opportunities for improvement and fold these into action planning for the
next year.
Attend Annual Executive Educational Forum: Enjoy your successes
and find out "Lessons Learned" from others!

Notes to Fee Schedule: Fees listed in Schedule do not include $100 non-refundable
fee due with Letter of Intent (see above).
High Plains letter of intent fee is $25.
- Site visits are automatic for Foothills, Timberline,
and Peak applicants. Site visit charges are set to cover reasonable costs
for travel, lodging, and meals and are in addition to the Application Fee.
- Not-for-Profit organizations include non-profit
organizations, healthcare not-for-profits, military, government, and schools.

2006 Award Recipients
Timberline Award Recipients/Applicants
LexisNexis Case Law and Citations in Colorado Springs is a past Foothills award
recipient and now a Timberline award recipient in 2006. As one of the top legal
research organizations in the world, LexisNexis is lead by Alison Manchester.
McKee Medical Center in Longmont is a high performing healthcare provide as
demonstrated by it s Timberline award in 2006. McKee is part of the Banner Healthcare
system and is lead by CEO Rick Sutton.
North Colorado Medical Center in Greeley is a past Foothills
award recipient and has demonstrated excellent deployment as a Timberline award
recipient in 2006. Gene O'Hara is the CEO at NCMC. NCMC is also a part of the
Banner Healthcare system.
Foothills Award Recipients
Emerson School in Grand Junction is a Foothills award recipient and is a member
of Mesa County Valley School District #51. Terry Schmalz is the principal and
Dr. Tim Mills is the superintendent in District #51.
Pulte Mortgage in Englewood is a Foothills award recipient in 2006. This organization
has demonstrated the first step towards performance excellence as the primary
mortgage lender for Pulte Homes. Debra Still is the CEO.
West Middle School in Grand Junction and a member of Mesa
County Valley School District #51 has demonstrated sound approaches and is a
Foothills award recipient. Vern Walker is the principal of West and Dr. Tim
Mills is the Superintendent of District #51.
Other Applicants Not Eligible for an Award
Poudre Valley Health Systems in Ft. Collins won the Peak award in 2004 and is
not eligible again until 2008 for an award. The CEO is Rulon Stacy.
Mesa County Valley School District #51 in Grand Junction and
Palisade won the Foothills award in 2003 and is not eligible again until 2007
for an award. The superintendent is Dr. Tim Mills.

2005 Award Recipients
Timberline Award Recipient
Veterans Administration Health Administration Center in Denver
is Colorado Performance Excellence's second ever Timberline award recipient demonstrating
excellence in deployment. The award was presented on October 12, 2005, by Lieutenant
Governor Jane Norton. Ralph Charlip is the Director of the VAHAC.
Foothills Award Recipients
Lexis Nexis Case Law and Citations in Colorado
Springs is a legal research and information organization demonstrating
excellence in approaches. Alison Manchester is the Vice-President
in charge of this high performing organization.
Mesa County Valley School District #51 boasts
its second and third Foothills Award recipients in the last
three years. Orchard Avenue Elementary School in Grand
Junction demonstrated excellence in approaches and best practices
in parent involvement. The leader of the school is Principal
Denise Hoctor.
Taylor Elementary School in Palisade is
high performing in approaches and best practices in accountability
and achievement. The leader of the school is Principal Dee
Crane.
North Colorado Medical Center in Greeley
is one of two healthcare Foothills award recipients in 2005.
This organization is a part of Banner Health and the only
hospital in Weld County. Their leader is CEO Gene O'Hara.
Spalding Rehabilitation Center in Aurora
is lead by CEO Cynthia Kreutz. Spalding provides best practices
in patient rehabilitation time to recovery and release and
demonstrates sound approaches. It is the second healthcare
recipient in 2005 and a member of Healthone.

2004 Award Recipients
Peak Award Recipient - First in CPEx History
Poudre Valley Health System in Ft. Collins
is Colorado Performance Excellence's historic first Peak Award
recipient. This award is equivalent in requirements to the
Malcolm Baldrige National Performance Award. All of the CPEx
awards were presented on October 21, 2004, at the third annual
CPEx Quest for Excellence III. Approximately 300 leaders from
organizations throughout the state attended the Conference.
The CEO is Rulon Stacey.
Timberline Award Recipients
- Transportation Expansion (T-REX) Project
Transportation Expansion (T-REX) Project is one of the historic
first two Timberline Award recipients for 2004. The Timberline
is awarded for approach and deployment in application of
the CPEx criteria for performance excellence. The headquarters
of T-REX is in Centennial, Colorado. The project director
is Laurence Warner.
- Monfort College of Business, University of Northern
Colorado
Monfort College of Business is one of the historic first
two Timberline Award recipients for 2004. The Timberline
is awarded for approach and deployment in application of
the CPEx criteria for performance excellence. The Monfort
College of Business is located in Greeley, Colorado. The
dean is Dr. Joe Alexander.
Foothills Award Recipients
- Arapahoe/Douglas Works!
Arapahoe/Douglas Works in Aurora is one of this year's Foothills
Award Recipients. Pat Buys is the Director. The Foothills
is awarded for achievement in approach in application of
the CPEx criteria for performance excellence.
- CAP Logistics
CAP Logistics in Denver is one of this year's Foothills
Award Recipients. Gayle Dendinger is the CEO. The Foothills
is awarded for achievement in approach in application of
the CPEx criteria for performance excellence. You may download

2003 Foothills Award recipient
Mesa County Valley School District 51 is this year's recipient
of the Colorado Foothills Award for performance excellence.
The award was presented October 2, 2003, at the second annual
CPEx Quest for Excellence conference, at the Marriott Denver
Tech Center. More than 225 leaders from organizations throughout
the state attended the conference.
2002 Foothills Award Recipient
IBM Global Services–Xcel Energy Account won the first Colorado
award for performance excellence at the Foothills Award level
for 2002. The award was presented October 10, 2002 at
the inaugural CPEx Executive Education Forum that was attended
by nearly 200 executives from companies and other organizations
throughout the state.
The performance excellence approach outlined by IBM Global
Services is highlighted by an integrated, pioneering
business relationship between IBM and Xcel.

work comes together to win the award. Finally, go to the Contact
